REMINDER: Physical day is THIS WEDNESDAY, May 15th at 3:30 PM in the BJHS gymnasium. In order for your student to get a physical, they must bring 2 things with them: 1. the 2024 UIL physical exam form with the medical history portion filled out and signed by a parent (this is your consent for your student to be evaluated by our doctors), and 2. $20 to cover the exam fee. Students who do not bring their completed medical history form will NOT be seen by the doctors. There are 5 stations your student must visit for their physical to be considered complete: Check In, Height/Weight, Blood Pressure, Vision Screening, Ortho Exam, and Check Out. The students will turn their completed physicals in at the check out station so that they can be filed for next school year. Your student SHOULD NOT bring home their completed physical exam form.
